As a recipient of grant funds received from the Nebraska Department of Roads (NDOR) for the purchase of a van for East Park Villa resident transportation purposes, Memorial Community Health is required to post the following procedures and forms relating to compliance with Title VI of the Civil Rights Act of 1964.
