Patient FAQs

How do I establish a primary care provider?

Please call 402-694-3191 for assistance in establishing a primary care provider. Information about our physicians and advanced practice medical providers is also available online to assist you in selecting a care giver that is best suited to meet your individual healthcare needs. What are my provider’s hours?

To check the availability of your primary care provider or to schedule an appointment, please call the clinic of your choice during regular business hours: Aurora: 402-694-3191; Clay Center: 402-762-3236. You may also Schedule Online at your convenience.

How do I cancel an appointment?

Memorial Community Health asks that any cancellations be made at least 24 hours in advance of the scheduled appointment in order that the time slot may be made available to other patients. Cancellations may be made by calling the appointment phone number for your clinic location: Aurora: 402-694-3191; Clay Center: 402-762-3236.

What time should I arrive for my appointment?

Memorial Community Health asks that new patients arrive 15-30 minutes prior to their scheduled appointment to allow adequate time for completing the registration process. For subsequent visits, patients are asked to arrive 5-10 minutes in advance of their scheduled appointment.

Who do I contact about a medical question?

Please call Memorial Health Clinic at 402-694-3191 during regular business hours and speak to a nurse. If the nurse is not able to answer your questions, he/she will consult with a provider and return your call. If you have a question outside of the clinic’s regular business hours, please call Memorial Hospital at 402-694-3171.

What should I do if I need treatment outside of the clinic’s regular business hours?

If you are sick outside of clinic hours and believe you may be in need of treatment, please call the Memorial Hospital Emergency room at 402-694-3171 and ask to speak to a nurse who will assess your situation and advise you with regard to immediate care. You may be advised to go to the Memorial Hospital Emergency room.

How do I locate a specialty physician?

Please refer to the Specialty Clinics page of our website for a listing of all the specialists who provide services at Memorial Hospital in Aurora.

Do I need an appointment to see a specialist at Memorial Hospital?

Yes. For information on where to call to make an appointment, please visit our Specialty Clinics page and click on the Specialty Clinic of your choice. In most cases, appointments are made by calling our hospital scheduling office at 402-694-8274, Monday-Friday, between 8:00 AM and 4:30 PM to schedule a time to be seen.

How do I find out about test results?

Test results may be delivered by telephone, mail, or be available in your My MCH Patient Portal. If you have a preference of how you would like to receive your results, please inform your provider. If you have not received your test results within the timeframe indicated by your provider, please call 402-694-3191 and inquire with your provider’s nurse. 

Please note that you may access your medical records, obtain test results, message your provider, and more through our Patient Portal. If you have issues logging in or signing up, or have multiple members in the same household, please contact our clinic manager at 402-694-8176 for assistance.

How do I get refills of my medication?

Please call your pharmacy during regular business hours to have your medication refilled. The pharmacy will communicate with your provider if there are questions or concerns. Please note that narcotic medications require approval by your individual ordering provider in order to refill. Therefore, requests to refill a narcotic can only be completed on days your ordering provider is in the clinic. Please plan ahead and allow 24-48 hours for refills.

What if I can’t afford medications that have been prescribed for me?

Medications can be provided through the individual pharmaceutical company at no or reduced cost to patients who meet certain eligibility requirements. Please contact a nurse during regular clinic business hours at 402-694-3191 to inquire.

What if I can’t afford necessary medical care or treatments?

No one requiring emergency care is ever turned away from Memorial Hospital due to an inability to pay. Both Memorial Hospital and Memorial Health Clinic will work with you to develop an individual payment plan, research loan options, apply for Medical Assistance, or arrange for reduced or free care (for patients who meet certain eligibility requirements). Please contact Hospital Accounts at 402-694-8208 or Clinic Patient Accounts at 402-694-8176 to receive additional information on which program is right for you.

Who do I contact if I have questions regarding insurance and billing?

Please refer to the Billing FAQs or Billing Information for more information regarding Memorial Community Health general billing procedures and to locate contact information for customer service representatives capable of answering your specific questions.

How can I get a copy of my medical record?

Patient medical records are the property of Memorial Community Health. You can review your records and be provided with copies (at a reasonable fee). To request copies or to make an appointment to review your medical record, please print out and complete the Release of Information Authorization Form and mail, email, or fax to:

How do I learn more about advanced care planning or end-of-life care?

Advanced care planning is available through our Connected Care program at Memorial Health Clinic. Call 402-694-8331 for more information. 

What information do I need to bring with me for Hospital admission?

Patients should bring the following information:

  • Insurance Card
  • Medicare/Medicaid card (if applicable)
  • Name and Social Security Number of insured
  • Referral forms from your physician (if required by your insurance carrier).
  • Worker’s compensation information (if applicable)
  • Driver’s license
  • Co-pay or deductible
  • Pre-admission form (if not previously submitted) 

What arrangements do I need to make with my insurance company prior to treatment?

Individual insurance programs vary. Please contact your insurance carrier regarding questions that you have with your plan. Many insurance companies require prior authorization for surgical procedures, hospital admissions, and diagnostic procedures (MRI, CT Scans, etc.). Memorial Community Health will gladly assist you in clarifying your carrier’s requirements, but you ultimately bear the responsibility for ensuring that you meet your insurance company’s guidelines.

Who do I contact if I have a general question about my hospitalization?

Please call the Memorial Hospital Nurses Station at 402-694-8235 for any questions or concerns that you might have regarding your hospital stay.

What should I pack for my hospital stay?

Patients are advised to bring photo identification, a copy of your insurance card, toiletries, sleepwear and personal items such as eyeglasses and dentures. It is also recommended that you bring a complete list of all medications including dosage and frequency with you.

How do I designate someone to speak on my behalf about my care?

As part of the admissions process, you will be given the opportunity to appoint someone with whom your physician or advanced practice medical provider can discuss your care. In order to have someone make medical decisions on your behalf if you are unable to communicate, you will need to complete a Healthcare Power of Attorney. It is advisable to be familiar with this and other legal documents such as a Living Will.

Who is my attending physician?

Your primary care physician will serve as your attending physician for any hospitalization. If your doctor is not available during your hospital stay, another Memorial Community Health family physician will be designated as your attending physician and will oversee your care.

What if I have a concern about my medical care?

Throughout your hospital stay you are encouraged to discuss your care with your attending provider. You may also request to speak with the Hospital Administrator or Director of Nursing about any concerns that you may have with your treatment plan or hospital stay.
